Consult Dr. Ash
TL;DR30-second take

Fishtown Medicine is the integrative primary care home for Magpie Midwifery families. Dr. Ash handles pre-conception optimization, postpartum recovery, hormone health, and longevity medicine, while your midwife leads pregnancy and birth. Long appointments, thorough labs, shared decision-making, and direct access to your doctor.

You chose a midwife because you believe birth is a normal physiological process rather than a medical problem. You chose providers who listen. You chose a community that honors the wisdom of your body. Your primary care physician should share those values.

I'm Dr. Ash, a board-certified internist, the founder of Fishtown Medicine, and a proud partner of the Magpie Midwifery community. I practice a model of medicine built on long appointments, close relationships, and the belief that you are the expert on your own body. I want to work alongside you as a teammate: you lead on your own body, and I bring the medical tools.

What I do: Integrative primary care, hormone optimization, pre-conception and postpartum support, metabolic health, and longevity medicine.

What I don't do: Pregnancy, obstetrics, or delivery. That is your midwife's lane, and she is extraordinary at it. I am here for everything around it.

Why do Magpie families choose Fishtown Medicine?

Magpie families choose Fishtown Medicine because we share the same care philosophy: unhurried visits, shared decision-making, and seeing you as a whole person. The difference is scope. Where your midwife guides you through pregnancy and birth, I handle the medical landscape that surrounds it.

If you have experienced Samm's care at Magpie, you know what it feels like to be heard all the way through. We practice medicine the same way.

  • Before conception: Optimizing your hormones, thyroid, nutrient stores, and metabolic health so your body is ready to conceive, rather than only stopping birth control and hoping.
  • Between pregnancies: Managing the metabolic changes, nutrient depletion, sleep disruption, and hormonal recalibration that postpartum life brings.
  • Beyond the childbearing years: Navigating perimenopause, cardiovascular risk, and the long arc of your health with the same intention you brought to birth.

What does Fishtown Medicine offer the Magpie community?

Fishtown Medicine offers the Magpie community a full range of integrative primary care services that complement midwifery, not replace it.

Pre-Conception Optimization

We call it soil before seed. Most doctors tell you to "try for 12 months" before investigating. We think that is backwards. We run a full diagnostic, including thyroid function, iron and ferritin (a measure of stored iron), vitamin D, metabolic markers, and hormone levels, then we optimize before you start trying. Your midwife manages the pregnancy. We help you build the foundation.

Postpartum Recovery and Nutrient Repletion

Growing a human is metabolically expensive. We see new parents months or years after birth still running on empty: depleted iron, crashed vitamin D, low thyroid function, hormones in disarray. We test, we identify the gaps, and we replete strategically.

Hormone Health Across Your Life

From PCOS to perimenopause to full menopause, hormones shape how you feel, think, sleep, and heal. We prescribe bioidentical hormone therapy (BHRT, hormones molecularly identical to what your body makes) when the data supports it. We push back on the "your labs are normal" dismissal that so many women hear. If you do not feel right, your labs may not be optimal. We dig deeper.

Relevant reading: Women's Hormone Health | Perimenopause | PCOS | Menopause 3.0

Metabolic and Longevity Medicine

We track what most doctors do not: insulin resistance years before diabetes shows up, ApoB and Lp(a) for a clearer picture of cardiovascular risk, inflammatory markers, and the metabolic changes that accelerate aging. This is prevention that gets ahead of problems instead of waiting for them to arrive.

Nervous System Support

A dysregulated nervous system is a medical issue worth treating. Whether it shows up as postpartum anxiety, chronic stress, or trauma stored in the body, we bring in somatic tools (physiological sighs, vagal toning, co-regulation strategies) alongside medical care when needed. Healing tends to happen in connection with other people, and we build that into the plan.

Relevant reading: Nervous System Regulation

Affirming, Trauma-Informed Care

Every body is welcome here. We use chosen names and pronouns, we practice proportional inquiry (asking only what we need), and we create a clinical environment where you never have to brace yourself. Care should feel safe.

Relevant reading: Affirming Care Standards | Trauma-Informed Care

How is this different from typical primary care?

This is different from typical primary care because the structure of the visit, the lab targets, and the access are all built around partnership instead of throughput.

How do we complement your existing team?

We complement your existing team by adding the medical bridge between holistic providers and the prescription pad. You probably already work with providers from the Magpie community: a chiropractor, an acupuncturist, a pelvic floor therapist, a naturopath. That is wonderful. We do not replace any of them.

What we add is the medical bridge: ordering imaging, prescribing bioidentical hormones, interpreting complex bloodwork, and catching the things that need an MD's scope, while speaking the same language your holistic team speaks.

Think of it as having a doctor who can write the prescription and understand why you would rather not take it unless it is clearly necessary.

Actionable Steps in Philly

Build the medical side of your Magpie support team.

  1. Book a free 15-minute call. We see if we are the right fit.
  2. Bring your latest labs. We review them through an optimal-range lens rather than a disease-only lens.
  3. Map the next 12 months. Pre-conception, postpartum, perimenopause: tell us where you are, and we build a plan around it.
  4. Stay connected by text and video. Same-day or next-day access for new questions.

The Membership

Fishtown Medicine is a Direct Primary Care (DPC) practice, a membership model not based on insurance billing. That means:

  • No copays, surprise bills, or insurance middleman.
  • Unlimited visits, in person, video, or text.
  • Same-day or next-day access when you need it.
  • Home visits available in Philadelphia.
  • Telehealth in 39 states for when life takes you beyond Philly.

We offer accessible pricing tiers so this model can reach regular families rather than only the wealthy.

A Note From Dr. Ash

I spent years in hospitals watching what happens when prevention fails: when the heart attack arrives, when the cancer is stage IV, when the autoimmune disease has been slowly building for a decade without anyone looking. That experience did not make me cynical. It made me focused on intervening earlier.

I founded Fishtown Medicine to practice the kind of medicine I would want for my own family: unhurried, thorough, grounded in evidence, and deeply respectful of the person across from me.

The Magpie community represents what I admire in healthcare: autonomy, trust, presence, and the belief that the person receiving care should be an active participant in it. I am honored to be part of your village.

Frequently Asked Questions

Common Questions

No, Fishtown Medicine does not deliver babies. Pregnancy and birth are your midwife's lane. We handle pre-conception optimization, postpartum recovery, and the rest of your medical life around the birth itself.
Fishtown Medicine works with Magpie Midwifery as a parallel partner. Your midwife manages pregnancy and delivery. We handle primary care, hormones, metabolic health, and longevity. We share notes when needed and respect your autonomy as the lead on your own care.
No, you do not have to use insurance with Fishtown Medicine. We are a Direct Primary Care (DPC) practice with a flat membership fee and no copays, surprise bills, or insurance middleman. Your insurance still pays for outside imaging, specialists, and prescriptions.
Yes, you can see Dr. Ash whether or not pregnancy is on your map. Many Magpie community members come for hormones, perimenopause, metabolic health, or general primary care that respects how they want to live.
Yes, we offer telehealth in 39 states. Many of our visits are virtual, particularly follow-ups. We also do home visits in Philadelphia and traditional office visits in Fishtown.
Yes, we will prescribe bioidentical hormones when the data supports it. We pair the prescription with full lab work, a careful history, and ongoing monitoring. We never write a hormone prescription on a single visit without a full picture.
You can usually get an appointment same-day or next-day for active concerns. New patient intakes are 90 minutes and scheduled within one to two weeks. Text and video access is included with your membership.
We focus on adult primary care. For pediatric care, we refer within the Magpie network of trusted pediatric providers. We do work with adolescents in some cases, particularly around hormones and metabolic health.

Deep-Dive Questions

"Soil before seed" is our pre-conception philosophy. Instead of waiting 12 months to investigate, we run thyroid, iron, ferritin, vitamin D, metabolic markers, and hormone panels months before you try. We optimize the foundation so the pregnancy begins in well-prepared metabolic ground.
Postpartum nutrient depletion goes undetected because standard postpartum care ends at six weeks and rarely includes thorough lab work. Iron, vitamin D, thyroid function, and B vitamins can stay depleted for years. We test the whole panel and replete strategically.
Perimenopause overlaps with the postpartum years for many women who have children in their late 30s or early 40s. The exhaustion, anxiety, and sleep changes can blur together. Testing helps separate hormonal changes from postpartum recovery, so we can treat the right thing.
Bioidentical hormone therapy (BHRT) uses hormones with the same molecular structure as the ones your body makes, including 17-beta estradiol, micronized progesterone, and testosterone. The data on transdermal estradiol and oral micronized progesterone is favorable, and we follow current consensus guidelines.
We test instead of treating perimenopause symptoms blindly because the data gives you agency. Seeing your low progesterone or changing FSH validates your experience and informs the dose. Some clinicians skip testing because levels fluctuate. We test thoughtfully and treat with both numbers and how you feel.
Insulin resistance affects fertility by raising insulin levels, which can drive the ovaries to overproduce testosterone, disrupt ovulation, and affect egg quality. Treating insulin resistance with diet, movement, and sometimes targeted medication often restores regular cycles within months.
Vitamin D supports immune function, mood, hormone production, and skeletal health for both parent and baby. Most adults in Philadelphia are deficient, particularly in winter. We test and replete to a target around 50 to 70 ng/mL during the pre-conception and postpartum windows.
We handle anxiety and nervous system regulation by combining medical care with practical somatic tools. Medication is one option. Breathing practices, vagal toning, sleep restoration, and hormone optimization are also high-value. We personalize the plan to what fits your life and your beliefs.
Direct text access is part of the practice because most patient questions do not need a full visit, just a quick answer. Texting your doctor saves time, calms anxiety, and prevents small issues from becoming bigger ones. It is included in your Fishtown Medicine membership.
Functional medicine is a systems-based clinical approach that asks why a problem started rather than only how to suppress symptoms. It uses standard medical training plus deeper lab interpretation. Naturopathy is a separate licensed profession. We work alongside naturopaths in the Magpie community when their tools match your goals.
We handle thyroid disease in pre-conception by aiming for tighter ranges than the standard reference. We target TSH (thyroid-stimulating hormone) under 2.5 mIU/L for conception and check free T3, free T4, and TPO antibodies (a marker of Hashimoto's thyroiditis). Treating thyroid early lowers miscarriage risk and supports fetal development.
Fishtown Medicine works with the Magpie community specifically because the Magpie philosophy of autonomy, trust, and presence aligns with how we practice. Long visits, shared decisions, and respect for your role as the expert on your own body are not common in modern medicine. We are honored to be part of the village.
