If your cholesterol came back normal and you were sent home with a plan to recheck it next year, the panel you had probably left out the numbers that predict the most. Cardiology has been asking primary care to update the lipid panel for more than a decade. The European cardiology guidelines moved to ApoB-preferred targets in 2019, and the American guidelines now list ApoB and Lp(a) among the markers it makes sense to measure. Most primary care offices in Philadelphia still order the standard panel and stop there.

Advanced lipid testing in Philadelphia is easy to get and inexpensive. What's harder to find is a physician with the time to read it against your family history, your blood pressure, and the rest of your life, and then change the plan because of what it says.

What's in the standard panel and why it's incomplete

When you get a cholesterol panel at most primary care offices in Philadelphia, the report comes back with:

  • Total cholesterol
  • LDL-C (low-density lipoprotein cholesterol)
  • HDL-C
  • Triglycerides
  • Calculated non-HDL cholesterol

LDL-C is the number everyone anchors to, and it carries 2 problems. It's usually calculated from the other values using the Friedewald or Martin-Hopkins equation rather than measured on its own, so it inherits whatever the equation assumes about you. And it describes how much cholesterol your LDL particles are carrying, while the thing that builds plaque is how many particles you have.

That's why 2 people can walk in with the same LDL-C and carry very different cardiovascular risk. If one of them has many small particles and the other has fewer large ones, the LDL-C reads the same while the biology underneath is nothing alike. ApoB settles that question by counting the particles directly.

ApoB: the marker that actually predicts risk

Apolipoprotein B, or ApoB, is a protein that rides on every particle capable of causing plaque: LDL, VLDL, IDL, and Lp(a). Every one of those particles carries a single ApoB molecule, so when the lab measures your ApoB it hands you a direct count of the particles driving atherosclerosis.

The work from Sniderman and Jukema, alongside several other large datasets, shows ApoB outperforming LDL-C at predicting who goes on to have a heart attack or a stroke. The European Society of Cardiology wrote ApoB-preferred targets into its guidelines in 2019, and the American guidelines now allow ApoB in place of LDL-C.

The target we aim for depends on your risk:

  • Low risk: under 90 mg/dL
  • Moderate risk (most adults): under 80 mg/dL
  • High risk (existing cardiovascular disease, diabetes, multiple risk factors): under 65 mg/dL
  • Very high risk: under 50 mg/dL

Those numbers are more assertive than what you'll hear at a standard visit, and they're where the current evidence points.

Lp(a): the once-in-a-lifetime test most patients have never had

Lipoprotein(a), or Lp(a), is a particle you inherit: an LDL-like core with a second protein, apolipoprotein(a), bound covalently to it. When your Lp(a) runs high, your risk of heart attack, stroke, and aortic valve disease rises on its own, independent of everything else on the panel.

The key facts:

  • Lp(a) is largely genetic. Your level is set in childhood and barely moves across adult life.
  • Diet, exercise, and standard lipid-lowering therapy don't change it much.
  • About 20% of adults have an elevated Lp(a), meaning above 50 mg/dL or 100 nmol/L.
  • Most people have never had it drawn.
  • When it's high, it changes the plan: a lower ApoB target, closer attention to every risk factor you can move, and earlier screening for aortic valve disease.

The recommendation is to test Lp(a) at least once in every adult's lifetime, and we draw it on every new adult patient at Fishtown Medicine. Since the number holds steady for life, a single draw is all you'll ever need.

Other advanced lipid measurements

LDL particle number (LDL-P) by NMR. This counts your particles using nuclear magnetic resonance, which makes it a close cousin of ApoB in concept. We usually prefer ApoB, since it captures every atherogenic particle rather than the LDL ones alone.

Particle size (small vs. large LDL). Smaller particles do more damage to the artery wall, so the split has value in some situations. ApoB already carries most of that information, so we rarely need to order it on its own.

hsCRP. High-sensitivity C-reactive protein measures low-grade inflammation, and when it runs high your cardiovascular risk rises independent of your lipids. We include it in the panel we run.

Homocysteine. A modest independent risk factor, and one that B vitamins can treat, so we add it when the rest of your picture makes it useful.

Cardiac imaging, after the upstream read. We'd rather read your ApoB, particle fractionation, Lp(a), and family history first, then decide whether imaging adds anything. A CAC scan (coronary artery calcium) sees calcified plaque and nothing else, and it runs roughly $150 cash at most Philadelphia imaging centers. CCTA (coronary CT angiography, increasingly read with Cleerly AI analysis) is the more sensitive step, because it also picks up soft plaque - the kind that ruptures and causes most heart attacks. If your cardiovascular risk falls in the intermediate range, one of these is often the right next move.

Where to get advanced lipid testing in Philadelphia

LabCorp and Quest both run ApoB, Lp(a), and the rest of the advanced lipid markers, and both keep draw stations across the city: Center City, Fishtown, University City, the Northeast, and out into the suburbs. Wherever you live, a draw site is a short trip away.

Insurance coverage:

  • ApoB: increasingly covered as a standard lipid marker.
  • Lp(a): increasingly covered, particularly when family history of cardiovascular disease is documented.
  • hsCRP: usually covered with appropriate indication.

How advanced lipid testing fits into Fishtown Medicine's preventive workup

Every adult in the practice gets an ApoB at the first preventive visit, then again on a schedule while we're managing your cardiovascular risk. Lp(a) gets drawn once and that's it for life. hsCRP comes along in the standard panel.

We set prevention targets off ApoB rather than LDL. When the two disagree, we follow the ApoB and leave the LDL number where it is.

If your Lp(a) comes back elevated, or something else in the advanced panel is concerning, we go further: a lower ApoB target than you'd otherwise get, a CAC scan to see how much atherosclerosis has already built up, closer work on every risk factor you can move, and in some cases earlier screening for valve disease.

We use a composite picture: ApoB level, Lp(a) level, family history, age, blood pressure, smoking, diabetes, kidney function, and CAC score (if available). For patients with multiple elevated risk factors, we go more assertive on ApoB targets and consider statins, ezetimibe, or sometimes PCSK9 inhibitors earlier than guidelines might suggest.
CAC scanning answers a different question: do you have evidence of atherosclerotic disease right now? A patient with elevated Lp(a) but a CAC score of zero has a different risk profile than one with elevated Lp(a) and a high CAC. The combination of advanced lipids plus CAC is often the highest-information cardiovascular workup available outside of advanced imaging.
LabCorp and Quest both run all the advanced lipid markers and are widely available throughout Philadelphia. Insurance coverage has been expanding. The bottleneck is primary care ordering: many physicians have not updated their default panel and many do not have time to interpret advanced lipid results. A direct primary care practice with the time fills this gap.
